1. Under the Forms tab, select the My Forms menu. Select Rename from the drop-down menu below the Edit button next to the form which you want to rename.
2. Under the Forms tab, select the My Forms menu. Click the name of the form you want to rename in the list. On the form editing screen that pops up, click on the "Form name" and rename form.
Note: The form name is only information you can see. Customers participating in the survey will not see the form name. For that reason, you can give the form name in a distinctive way for yourself.
